Thank you for your interest in our hunting and fishing trips and for supporting our Mission! To participate in one of our upcoming activities we require completion of the below application/waiver form.
Waiver/Application Form
In exchange for participation and use of the property and services with the Honor and Valor Outdoors Hunting/Fishing Program, organized by Honor and Valor Outdoors, agree to the following:
AGREEMENT TO THE FOLLOWING DIRECTIONS. I agree to observe and obey all laws and rules provided by the Wisconsin Department of Natural Resources regarding the hunting/fishing regulations, and further agree to follow any oral instructions or directions given by members of Honor and Valor Outdoors.
ASSUMPTION OF THE RISKS AND RELEASE. I recognize that there are certain inherent risks associated with the hunting/fishing provided and I assume full responsibility for personal injury to myself and others. I am aware that any unsafe act will result in immediate cancellation of the hunt and termination of future participation with Honor and Valor Outdoors activities.
LAND USE AND TRESPASSING. I am aware that property used to provide Honor and Valor Outdoors hunts are used for the sole purpose of providing Honor and Valor Outdoors activities. I acknowledge that at no time after the hunt will I trespass onto property used for these activities. Trespassing will result in police intervention and termination from future Honor and Valor Outdoors activities.
NO DURESS. I agree and acknowledge that I am under no pressure or duress to sign this agreement and that I have been given a reasonable opportunity to review it before signing.
